We've been able to reduce chemical restraints in the patient population, and improve patient safety and quality of life by using our “Behavior Health Evaluation Tool Kit” which incorporates a behavior management meeting template, drug information, dosing recommendations, and clinically relevant information from the patient’s health care providers. We've also collaborated with the Area Agency on Aging (AAA) to develop a 4-alert system to improve AAA patient safety. The system evaluates a patient’s medication list of interactions, cognitive impairment, hospitalizations, and duplicate therapies.

What is Comprehensive Medication Management?
Comprehensive medication management is defined, by the Patient-Centered Primary Care Collaborative, as "the standard of care that ensures each patient's medications (whether they are prescription, nonprescription, alternative, traditional, vitamins, or nutritional supplements) are individually assessed to determine that each medication is appropriate for the patient, effective for the medical condition, safe given the comorbidities and other medications being taken, and able to be taken by the patient as intended. Comprehensive medication management includes an individualized care plan that achieves the intended goals of therapy with appropriate follow-up to determine actual patient outcomes. This all occurs because the patient understands, agrees with, and actively participates in the treatment regimen, thus optimizing each patient's medication experience and clinical outcomes."

Responsibilities of the consultant pharmacist include:
- Participate in the drug therapy decision-making process in nursing facilities and recommend appropriate drug and non-drug therapies for use in residents based upon the disease or condition under treatment, other medical problems, physiological changes related to the aging or disease process, and residents’ beliefs
- Design, implement, and monitor the pharmaceutical care plan, in conjunction with the resident’s overall plan of care, to ensure therapeutic and/or functional goals are achieved, identify and solve problems, and provide a clinical judgment as to the continuing effectiveness of individualized pharmaceutical care plans
- Initiate and modify drug therapy, and order and monitor vital signs and laboratory assessments in cooperation with the attending physician, in accordance with the facility’s policies and procedures and the state’s pharmacy practice act
- Assist the nursing facility in developing and implementing appropriate policies and procedures
- Assist the nursing facility in developing and maintaining a comprehensive disease management program and develop protocols for safe and effective use of medications using appropriate clinical tools such as practice guidelines, treatment algorithms, and protocols
- Oversee all aspects of pharmacy services in the nursing facility related to the medication use process and supervise medication management systems to ensure that adequate supplies of medications are available to meet resident care needs, stored under appropriate and secure conditions, delivered to residents in a timely, safe and efficient manner, and labeled appropriately
- Evaluate medication administration to verify that it conforms with the prescriber’s orders and the facility’s policies and procedures, and to ensure the facility is free of significant medication administration errors
